1. Except for passport number, you need fill exactly your full name, date of birth, nationality and estimated arrival date.
2. You need to prepare 2 passport 4x6cm photos and stamping fee with 25 USD in cash for single entry or 50 USD in cash for multiple entries. You can pay stamping fee by American dollar or British pounds.
